Application For Certification Of Military Natural Resources Professionals

INSTRUCTIONS: Application must be typed. If submitting an application by mail please use word processing, the entire page, including instructions, must be duplicated. Submit original, three (3) clearly readable copies, and appropriate fee payable to: NMFWA, Treasurer, 4421 S. Myrtle St, Spokane, WA 99223. It is the applicant's responsibility to provide full documentation of previous professional certification, supplemental military-specific education and military-specific professional-level experience. Failure to provide complete documentation may limit your rights of appeal if you are denied certification. Applications must be accompanied by four (4) clearly legible copies of previous professional certification certificates. (Do not send originals, as no copies will be returned.)
